Carbocisteine is a mucolytic that reduces the viscosity of sputum and so can be used to help relieve the symptoms of chronic obstructive pulmonary disorder (COPD) and bronchiectasis by allowing the sufferer to bring up sputum more easily. Carbocisteine reportedly scavenged H2O2, HOCl, OH+ and ONOO– in cell-free medium, inhibited ROS production in rat neutrophils, promoted GSH and HO-1 expression in cigarette smoke-exposed bronchial epithelial cells, and ameliorated oxidant-induced 16HBE cell apoptosis. Furthermore, carbocisteine was capable of attenuating the synthesis of pro-inflammatory cytokines following rhinovirus or respiratory syncytial virus infection. It was speculated that the efficacy of carbocisteine in COPD was more closely associated with its anti-oxidant and anti-inflammatory effects, than with mucolytic activity.
