Angiotensin-converting enzyme inhibitor
Cilazapril is a prodrug which is hydrolysed to the active angiotensin converting enzyme (ACE) inhibitor cilazaprilat after absorption. It prevents conversion of angiotensin I into angiotensin II causing an increase in plasma renin activity and a decrease in plasma angiotensin II and aldosterone concentrations. Most data support the hypothesis that the beneficial haemodynamic effects of cilazapril result from ACE inhibition and the consequent reduction of angiotensin II levels, which either directly or indirectly causes dilatation of peripheral vessels and reduction of vascular resistance. It prevents the reabsorption of sodium and water from renal tubules and decreases the heart flow rate.
